Submit Your Completed Entry Form

HOW TO SUBMIT THE FORM

• Complete the contest entry form in its entirety.
• Save the document.
• Upload the document using the upload form at the bottom of this page.
• You will receive an email confirmation that your entry was received.
• NOTE: Only files with a .doc, .docx, .rtf, .pdf or .zip format will be accepted.

If for some reason you are unable to get the file upload feature to work for you, you may email the completed Word document to jilledwards@psu.edu.

• Entries are accepted electronically ONLY. No entries will be accepted by regular mail.
• Only one application per business is acceptable. There is no limit to the number of separate businesses for which you can submit applications.
• All information and submissions are kept strictly confidential and will not be shared with any outside agencies. In addition, all contest reviewers have signed confidentiality agreements.
